Ozzie Smith, Greg Maddux, Johnny Bench, Ivan Rodriguez, Ken Griffey Jr.

This one isn't too hard (those aren't the only players that fit in the group)

Every position played in a single game? I seem to remember Campaneris doing it. And I know Jose Oquendo did it over the course of a season for StL.

EDIT: Just googled it, turns out I was wrong :*)

(But I was right about Campaneris playing every position)
