Currently in third place in a 12-team 6x6 h2h league w/ fielding percentage (negligible effect) and holds (forces more use of middle relievers). Still, I'm struggling to stay competitive as my team is being utterly ravaged by injury (in addition to the roster below I also had Kendrys Morales, Jose Contreras, Jonathan Broxton, Jason Bay and Grady Sizemore at various points this season).
Should I pick up Justin Smoak, Nick Markakis or Josh Willingham? If so, who could I even drop?

I don't see any of those being worth a speculative add, considering that you would have to drop someone. Of the three, Smoak has the greatest potential to return dividends, but I wouldn't drop Rizzo or Trumbo, or one of those similar "flier" players for him.
The only move I might be interested in would be dropping one of Trumbo or Rizzo for Smoak. But while I guess I like Smoak the best out of those three, I think they're all around the same value.
