Yahoo only has one confirmation screen, and it's easy to get in the habit of clicking through it without paying attention (especially after a night at the bar...lost Josh Beckett in a playoff run one year).

I've also reversed trades when the owner immediately emailed me and said he clicked the wrong button. Contrary to what someone else said, I'd much rather have an easygoing commissioner in these circumstances. Being too strict takes the fun out of the league.
