AT&T U-Verse, but as it has been pointed out, it's not cheap. On the flipside, it is extremely reliable and there are a ton of features. Multi-view (4 channels at once), lots of HD channels, being able to record and play back from the DVR in any room, etc. I tried calling them to get a better deal since they are so expensive (I've seen lots of posts from people with Comcast being able to get better deals simply by asking), but no dice on the AT&T front. Ah well.

I use TimeWarner Cable and Road Runner. It's expensive as hell and I only have one DVR. I think our bill for TV, Internet and Phone is somewhere around $165 a month, but there are no competitors here.
I tried DirecTV but I lost the signal constantly since we live just off the civilian side of the airport and when a plane passes between the dish and the satellite it went freaky. That happened at least once per day, and the storms we got that year knocked out coverage as well (when I really needed TV), so I won't go back to that.
I've always had Dish Network and just switched to Comcast last week, they were running a $29.99 for 12 months with free HBO special. I mainly switched because I want to hook up at least 6 TV's and with Dish you have to pay $5 a month per tv whereas it's free with Comcast.

We've got Cox here in Roanoke, VA and we're paying like $118 a month for internet and digital cable with 1 HD receiver and no DVR's. Some of you don't have it all that bad.

i pay about $155/month for Comcast HD DVR x2 extra digital outlet (yes those bastards charge for digital service per TV) HBO 1.0 MBS Internet (i can live with slow downloads for $15 less a month) modem ($5...i should just buy my own but i'm sure it would break or they'd upgrade and it would become obsolete)
if they ever bring U-verse into town i might switch but i know in the end it will end up costing just as much so i might as well just accept getting bent over and sticking with a service i like

$7.95 per month per TV if you want HD on more than one.
