I've been offered either Jason worth or Nelson Cruz and Pablo Sandoval for Derek Lee, does anyone think Lee will Keep up the numbers he has put up this month

Do you have a place to put everyone in the OF? And do you have 2 people you can drop/move in to cover those spots? I would do it if it doesn't handicap your team in other ways. If you're stacked in the OF already or there's no decent cornerman on the WW that can pick up the 3B corner, then I wouldn't trade away both corners like that.

i have markakis, justin upton, and mclouth, and hart in the out field now, and i have Cabrera and chipper for my corners if i do the trade

I think I'd do both trades. I'm not sold on Lee continuing his power and Zimmerman's lineup is just not strong enough.

You would still have MCab and Chipper? Then definitely do the deal. Rotate your OFs when they get cold or move a couple for upgrades elsewhere, like in your pitching or middle infield, if you're struggling in one of those areas.
