Larry Yount, brother of Robin Yount, made his debut when he was 21. He was called into the game, but got hurt while warming up on the mound, and was taken out before he threw a pitch. He still got credited with a game appearance, but he never made it back to the majors.
Then you got Doc Hamann who made his debut as a 21 year old. Came in and gave up 3 hits, 3 walks, hit a batter and thew a wild pitch. Made 0 outs and ended up with 6 earned runs. Never pitched again and ends with a career ERA of infinity.
And just as a bonus, a player named Lewis made his debut in 1890 for the last place Buffalo Bisons who played in the Players League. His line: 3 Innings pitched, 13 hits, 7 walks, 3 HRs, 20 earned runs. Finished with a career ERA of 60. He did however manage to strike out 1 guy and even got a hit.

Sent in to play the bottom of the 8th inning in the OF. He was left on deck in the top of the 9th inning when the third out was recorded. Never played in another Major League Game, and never got to bat. He would later get a chance to play again, w/ Shoeless Joe Jackson and most of the 1919 BlackSox players, but gave it all up to save a girls life that was chocking on a hot dog that became lodged in her air passage after falling off the bleachers. Sad, sad story...
Maybe not worst career but this year for the Brewers Brad Nelson went 0 for 21 with an .087 OPS before they released him and the Mariners picked him up. That is one of the worst seasons I've seen assuming he doesn't play in the majors again this year.

i seem to remember a cub named Adam Greenberg who got called up with Matt Murton late in the year, faced one pitch that hit him in the head, and hasn't seen the majors again. at least he's got a career 1.000 obp.
