Everyone just chatters all day and vote for someone to lynch
Mafioso get to kill someone every night
Investigator gets to find out the identity of one person every night
I have no idea how the doctor is involved

So... What does the doc do? No one gets to talk outside the game right? Or just the mafia?

Only the mafia talk to each other.

The doctor chooses one person to protect at the end of the day. If the mafia chooses the same person to kill over night then that person is saved by the doctor.

also do the families know each other and work together or are they trying to kill each other?

They know their partner but are competing against each other and do not know the other two.

Each will get a chance to kill at night.
