If this guy is the end all be all in arm surgery, why do so many guys not come back great from him? I understand it's a case by case basis, but is he that much of a pwn3r that nobody goes to anybody else for an opinon or a surgery?

I actually don't believe he exists. I believe "going to see Dr. James Andrews" is just a euphemism for "very bad hemorrhoids." Carlos Guillen is the first guy to just say no. Hemorrhoids are the real scourge of baseball, not steroids.
