The answer to your question is about as often as you see a Save and a perfect game.
Jose Valverde was mistakenly credited with both earlier this week, as I'm sure Casilla was mistakenly credited this time.

A hold is sometimes incorrectly credited and then corrected afterwards. The definition of a hold includes "does not finish the game" or similar wording...

Actually I am a dummy even though I was technically right, but for the wrong reason. I confused a win with a save when citing the definition of a hold. But you still can't have both because a hold is awarded when you protect a lead. A pitcher can't get a hold if he blows the lead. And since the only way to get a win in that situation is to blow the lead and have your team save your bacon, you are SOL on having both. anyone credited with both is done so incorrectly and should have it corrected.

We'll see if this shakes out but both Yahoo and CBS have given Casilla a HOLD and a WIN.
