#5 has been retired 9 times. First was by the Reds in memory of Willard Hershberger but they reactivated it in 1942 and then retired it again 40 years later for Johnny Bench.
First permantley retired number was Joe Dimaggio of the Yankees in 1952

The answer is the #5 retired first by the Yankees for Joe DiMaggio.

The both the number 4 and 5 currently is currently retired by 8 teams. The Cincinnati also retired the number 5 for the 1940-1941 season in honor of Willard Hershberger (giving it 9 total).
