Qualls got two outs last night but was credited with three Ks. The first batter he faced struck out but reached on a wild pitch (reported also as a passed ball), thus there was no out but the K was credited to Qualls. He went on to strike out two more batters, hence a line of 0.2 IP and 3 Ks.
I know it's not unheard of but it's not that common either. Incidentally, he also gave up an ER.

BJ Ryan was given a free strikeout last week:
His stat line was 1.0 IP and 2k's, but this is the inning summary for the game:
Bottom 9th: Boston
- B.J. Ryan relieved J. Carlson
- M. Ramirez struck out swinging
- M. Lowell flied out to center
- B. Moss walked
- C. Crisp flied out to shallow right center

Interesting. Either the stat line or the play by play for Ryan is incorrect, obviously. However, in the Qualls case I believe the three Ks are legitimate despite there being only two actual outs.

On the BJ Ryan thing.. Yahoo got it wrong. Crisp flewout to shallow right field on the same pitch Ryan balked.. So i think Moss was on 1st, he went to 2nd base. and Crisp was still at bat with the same count.. The very next pitch Ryan struck Crisp out..
so the game was over

I remember a few years back Mark Prior had an inning where he had 4 strikeouts (one reached 1st on a wild pitch)
