The testicle torsion I'd heard of before, a couple of times actually.

But I think this one crowns the 2008 injury list:

Houston Astros second baseman Kazuo Matsui will undergo surgery to repair an anal fissure on Monday in Houston.

Pie fell victim to a spring training 2008 injury bug that has afflicted the Cubs. Originally deemed "a minor surgery on the groin region," Cubs.com has reported the injury as "a twisted testicle." The surgery involves sewing the outer layer of the testicle to the scrotum wall. The surgery was deemed successful, and Pie reported back to spring training, but was to be held out for more than a week..

Basically, the testicle twists on itself and the veins and blood vessels get pinched off. Blood keeps going to the testicle, but can't leave the region. The testicle swells and swlls. It's about the most painful think a man can endure. How he played on it for a couple days is beyond me. It happened to me when I was 16. I barely drove myself to the hospital, and then I passed out in a wheel chair in the lobby of the ER. I woke up the next day with my nutz in a sling. Got my first bottle of vicodin that week though, and extra special attention from my girlfriend for a couple of months!!
