But seriously, how could the umpires not see a situation like this coming? It seemed like ultimately they weren't sure. If I was umpiring and that guy was on the roster, I would make sure that I knew the correct rules. It wouldn't have been that hard to contact whatever the highest authority on umpiring is.
So, what is the official ruling on this? I never figured out what the rule is in that situation. Was it that he has to see at least one pitch if he steps into the batters box?

It sounds like it boils down to that the pitcher has to say first. But at what point has he "declared"? Is it when he steps on the rubber? What if the batter steps in the box before he takes the rubber? Is the batter allowed to step out of the box and go to the other side? I haven't ever seen the exact text governing the situation, but I'd be interested in reading it if anyone has it.
