Your outstanding performance in this season's Yahoo! Sports GMC Fantasy Baseball PLUS competition has made you the potential recipient of one of our League Winner Prizes -- a championship plaque commemorating your achievement.

To accept the championship award you must meet the eligibility requirements (below) and confirm that the address you gave us during the game's registration was a valid U.S. shipping address.

As stated in the Official Contest Rules, to be eligible to receive a prize in this contest, you must be: a) at least 18 years of age; b) a U.S. resident NOT living in a jurisdiction where the contest is void (Arizona, Florida, Maryland, Vermont, Montana, Louisiana, New Jersey, Arkansas, Tennessee, overseas U.S. territories, possessions, commonwealths and military installations, and where otherwise prohibited by law); and c) have fully complied with the Official Contest Rules (http://help.yahoo.com/help/us/baseballp ... es-31.html) and the game's Additional Terms of Service (http://baseball.fantasysports.yahoo.com/b2/tos).

If you meet the eligibility requirements described above, please claim your prize by confirming (or correcting) your address information by following the instructions below:

1) Go to the Yahoo! Sports GMC Fantasy Baseball PLUS home page.
2) Go to your league-winning Team Page.
3) Click on the 'Edit Team Info' link.
4) Confirm (or correct) your address to be a valid U.S. shipping address (prizes will NOT be shipped to incomplete addresses, PO Boxes or military bases)
5) Click the Submit button

YOU MUST SUBMIT A VALID U.S. SHIPPING ADDRESS TO THE 'EDIT TEAM INFO' PAGE BY NOVEMBER 10, 2005. FAILURE TO PROVIDE AN ADDRESS BY THE ABOVE DATE WILL RESULT IN DISQUALIFICATION AND YOUR PRIZE MAY BE AWARDED TO THE NEXT ELIGIBLE RUNNER UP IN YOUR LEAGUE.
