At one point Had Sheff, Matsui, Giambi, Posada and Mussina on my team. I recently cut both Giambi and Posada. It doesnt matter in Roto since everything balances out, but in H2H it can kill you if that team has a week where they get shut-down
I try to avoid more than two hitters per team, but I don't mind adding another as the season progresses if I'm just going to ride that hitter's hot streak (like...say Jason Giambi).
As far as pitching goes, I would stay away from having more than one reliever from the same team if you play in a holds league. It's a bummer when Hoffman doesn't get a save opportunity for the week because Linebrink also doesn't get a hold opportunity. Starting pitchers are alright, except for the fact that every once in awhile you'll get two pitchers starting in Coors on back-to-back days and then you are relying on your non-aces to give you quality innings.
Now that's all if you are playing H2H...if you are playing roto, there's not too much risk involved except for the fact that if one of them goes down, it'll affect the other one's rbi opportunities and the like.
For the most part, I try to stay away from stacking up on one team (note: I have stacked up on Oakland A's as this season has progressed, but I only drafted one A).
I like the idea of stacking up on players from one team but it has to be an offensive juggernaut like the Yankees, Bosox or Rangers. Other than that it's a bad idea to have more than two...and even two is stretching it if they end up going up against a team with devastating pitching.
In H2H that could really hurt you, It has never been a problem for me though in roto leagues. In H2H if the team goes on a cold streak your going to suck for quite a bit, and though there might be hot streaks from those guys, since you have 4 guys from one team so the chances are more of them are going to be cold rather than hot most of the time.

I played Cameron, Beltran, and Floyd as my outfield for about a week much earlier in the season
